Welcome to Sumter Gastroenterology, LLC. We are pleased that you and your primary care physician have chosen us for your care. If you have an upcoming appointment, the following information will help make your first visit as comfortable as possible. • In an attempt to provide a smoother and timelier check-in, you will receive a packet of forms by mail prior to your visit. Please fill them out completely and bring them with your insurance cards and government issued picture ID card to your first appointment.
• Gastro Policies
  Please bring all of your medications that you are currently taking.
• Your insurance plan may require a referral. We ask that you contact your primary care physicians’ office to verify the referral has been completed prior to your appointment. We do have patients sign a waiver indicating they will be responsible for payment for the service if the referral has not been received.
